Eligibility

Would you like to find out if you or a family member is eligible to receive NLACRC services? First, look under Related Information to the right and see if you live in the catchment area that we serve (Antelope, Santa Clarita and San Fernando Valleys of Los Angeles County). If you do live in the NLACRC catchment area, you can begin the intake process by completing either an Early Start or Lanterman Services application (see below).

Early Start Program and Prevention Program (Ages 0-3)
An application must be completed in order to assist the North Los Angeles County Regional Center (NLACRC) to determine eligibility for the California Early Start Program. Eligibility for Early Start is based on an infant or toddler having one or more of the following conditions:

  • An Established Risk condition. This is a diagnosed medical condition which has a high probability of resulting in a developmental delay, and/or disability.
  • A one-third delay in one or more area of development before 24 months, or, at 24 months of age or older, either a fifty percent delay in one area or a one-third delay in two or more developmental areas.

If your child is not eligible for Early Start, your child will be considered for California’s Prevention Program, which provides developmental monitoring to children birth thru 35 months of age with high risk conditions, and certain developmental delays (children between 24 and 35 months of age with a
33-49% delay in one developmental domain) that do not otherwise qualify them for Early Start.

To determine your child’s eligibility, NLACRC will complete an evaluation to assess your child’s developmental level. Additionally, a review of your child’s medical history/records is required. This application contains the necessary forms required for NLACRC to initiate the evaluation process. Early Start eligibility determination may take up to 45 days.

Apply for the Early Start Program (ages 0-3)

You can submit the application one of two ways:

  1. Complete the application online and sign it using a mouse, (you must use Adobe Acrobat Reader Version 9). Send the saved document as an attachment by e-mail it to intake@nlacrc.org or you can request that we mail the application to you by providing us with your name and address.
  2. Complete the application, sign it, and send it by fax to the intake department at (818) 756-6357, or mail it to North Los Angeles County Regional Center, Attn: Intake Dept., 15400 Sherman Way, Suite 170, Van Nuys, CA 91406.

You can also visit any of the three NLACRC offices and fill out an application.

Lanterman Act Services (Ages 3+ through Adult)
An application must be completed in order to assist the North Los Angeles County Regional Center (NLACRC) to determine eligibility for services under the Lanterman Developmental Disabilities Services Act. To be eligible for Regional Center Services an individual must have a Developmental Disability as per California Law and Regulation.

A developmental disability is a condition attributable to:

  • Mental Retardation
  • Epilepsy
  • Cerebral Palsy
  • Autism
  • Disabling conditions found to be closely related to mental retardation or requiring treatment similar to.

Additionally, the disability must: Originate prior to the age of 18, continues or is expected to continue indefinitely and constitutes a substantially for the person. Substantially disability means significant functional limitation in three or more of the following areas of life activity: self care, receptive and expressive language, learning, mobility, self direction, capacity of independent living, economic self sufficiency. A developmental disability does not include other handicapping conditions that are solely physical in nature, solely psychiatric in nature and solely learning disabilities.

To determine the applicant’s eligibility, NLACRC will complete an intake assessment which may include collection of historical diagnostic information, such as medical records, school records, prior psychological testing as well as provision of diagnostic evaluation (s) if indicated. This application contains the necessary forms required for NLACRC to initiate the evaluation process. The applicant’s information is confidential and will only be released with your written consent. Please be informed that the evaluation process cannot begin prior to receipt of your written consent. Eligibility determination may take up to 120 days.

Prenatal Services
NLACRC may provide certain prenatal diagnostic services to pregnant women who are at high risk for giving birth to a child with a developmental disability and no generic resources (such as medi-cal or private health insurance) are available. Please call the Intake department at (818) 778-4400 for more information.
